This questionnaire form provides technical information used by the Contracts Office to prepare the final contract package. THIS IS NOT A CONTRACT DOCUMENT. Submit a completed copy of this questionnaire at the pre-final and final design submittal. This form must be completed by the A/E (Architect/Engineer) or the Government (PM or DM) depending on who the Designer of Record is.
